The Health Information Services (HIS), formerly know as Medical Records, is located on the first floor, adjacent to the WMH cafeteria. Patients may obtain only their own medical information and must sign a release in order to obtain records. Please contact the HIS and request records prior to coming to the hospital. By calling ahead, records and the HIS Release Forms can be retrieved and prepared prior to the date of pick-up, allowing for faster processing for patients.
